Hannah Tranchina is a Family Nurse Practitioner, certified by the American Academy of Nurse Practitioners and is a member of Sigma Theta Tau National Honor Society. She earned her Bachelors of Science in Nursing from Nicholls State University and her Masters of Science in Nursing at the University of South Alabama. She is an active member of Louisiana Association of Nurse Practitioners. Hannah joined South Louisiana Medical Associates in September 2015 and is currently practicing at START Community Health Center.
Hannah’s prior experience included Internal Medicine offering both inpatient and outpatient services. She began her career as a registered nurse at Thibodaux Regional Medical Center. She treated patients of all ages in the Intensive Care Unit as well as Medical- Surgical Unit. During her clinical experience, she gained experience in primary care, cardiac surgery, and many other important skills.
Through her experience in the critical care setting, Hannah has clinically responded to the effects of under-managed chronic diseases. In the outpatient care setting, she focuses on disease prevention and strives to emphasize the benefits that healthy choices and an active lifestyle have in optimizing health. She actively listens and educates her patients and families to help them achieve their healthcare goals.
In her free time, Hannah enjoys jogging and traveling. She enjoys servicing her community and is an active member of Thibodaux Service League. But, what she enjoys most is being with family – especially her husband and daughter.
